Social prescribing and the wider determinants of health

Overview:

As part of the NHS Long Term Plan, significant funding has been made available to recruit and train over 1000 Social Prescribing Link Workers, based in Primary Care. Support people with a wide range of social, emotional or practical needs many PCN’s have focused on working in partnership with voluntary and community sector organisations to address people’s needs in a holistic way. However, there is much more to this topic than meets the eye.

Our panel discusses the wider determinants of health and care, the role that other agencies and bodies play in creating a whole system approach to and how to make the most of the support available for PCN’s.
